DISTINGUISHED CITIZEN AWARD NOMINATION FORM

Over the years, the Brick Township Chamber of Commerce has presented Distinguished Citizen(s) Awards to those individuals, businesses and organizations that have contributed to making Brick Township a better place to live and work through their dedication and volunteer involvement. 

If you would like to make a nomination in any of the following categories, please complete the bottom portion of this page and return it to the Chamber office.

 1)  Distinguished Citizen - Someone who truly exemplifies outstanding community service through their volunteer activities.

 2)  Distinguished Organization - A non-profit organization that excels in their volunteer activities.

3) Distinguished Business - A business that consistently supports projects that benefit our Community.
